IMPORTANT E911 DISCLOSURE ADDENDUM FOR MAGi VoIP SERVICE VOICE OVER INTERNET PROTOCOL ("VOIP") CUSTOMERS
The Federal Communications Commission ("FCC") recently adopted an E911 VoIP Order that requires MAGi, like all VoIP service providers, to inform its customers of any differences between the E911 access capability available with MAGi VoIP service as compared to the E911 access capability available with traditional telephone service. It is important that you understand how these differences affect your ability to access E911 services. For example, in certain circumstances, you may not be able to contact emergency services by dialing 9-1-1 using MAGi VoIP service and must inform your MAGi VoIP service users of this.
We ask that you carefully read this E911 Disclosure Addendum. The FCC's E911 VoIP decision also requires MAGI to obtain and keep a record on file showing that your company has received and understood this E911 Disclosure Addendum. MAGI respectfully requests your assistance in meeting this FCC compliance requirement by checking the applicable box on the Service Order Agreement.

LIMITATIONS ON ACCESS TO E911 SERVICES
- YOU MUST MAINTAIN ALTERNATE MEANS OF CONTACTING E911 SERVICES. MAGI VoIP service E911 service differs from traditional E911 service. You understand and acknowledge that you must have alternate means of contacting E911 services and inform your MAGi VoIP service users of this. You will receive from MAGI a set of warning stickers explaining when E911 service may not be available. Please place the stickers on or near the equipment you use to access MAGI VoIP service. If you need additional stickers, please contact your Customer Service representatives at (614) 638-9959
- MAGIVOIP SERVICE EMERGENCY SERVICES MAY NOT OPERATE DURING A POWER OUTAGE. Although the Integrated Access Device (IAD) MAGI placed on your premises is equipped with back-up battery power, in the event of a commercial power outage lasting longer then the IAD battery life, the IAD will lose power causing a loss of voice and data service, including E911 services. You understand and acknowledge that you may not be able to use your MAGI VoIP service to contact E911 services if your power is disrupted. Once power service is restored, you may be required to reset or reconfigure your equipment before you will be able to use MAGI VoIP service to contact E911 services. You are responsible for providing an uninterruptible backup power supply to ensure continued operation of electrical equipment, including customer premises equipment necessary to provide telecommunications and E911 service, in the event of a power outage.
- MAGIVOIP SERVICE E911 SERVICES WILL NOT OPERATE IF YOUR BROADBAND CONNECTION IS DISRUPTED OR MAGIVOIP SERVICE HAS BEEN SUSPENDED FOR ANY REASON. You understand and acknowledge that you will not be able to use your MAGI VoIP service or contact E911 services if your broadband connection is disrupted or if MAGI VoIP service has been suspended for any reason, including, for example, for non-payment. Once your broadband connection and MAGi VoIP service have been restored, you may be required to reset or reconfigure your equipment before you will be able to use MAGi VoIP service to contact E911 services.
- MAGIVOIP SERVICE E911 SERVICES WILL NOT OPERATE UNLESS YOU REGISTER YOUR CORRECT SERVICE ADDRESS WITH MAGI. You understand and acknowledge that you must provide MAGI with your correct service address in order for MAGi VoIP service and E911 services to work. If you notice that the location information identified in your contract or bill is inaccurate, you can correct service address by calling our Customer Service representatives at (614) 596-9382 during the hours of 8 a.m. to 8 p.m. EST.
- MAGi VOIP SERVICE E911 SERVICES WILL NOT OPERATE CORRECTLY IF YOU DISABLE, DAMAGE OR MOVE THE INTEGRATED ACCESS DEVICE TO ANOTHER LOCATION. If you disable, damage or move the IAD to a location other than the service address you provided MAGI when service was initiated, it will not function. If you wish to move to a new service address, please call either your sales representative or Customer Service at 614-596-9382 during the hours of 8 a.m. to 8 p.m. EST.
- MAGi VOIP SERVICE E911 SERVICES CALLS MAY BE DELAYED OR DROPPED DUE TO NETWORK ARCHITECTURE. You understand and acknowledge that your calls to E911 services made utilizing MAGi VoIP service may take longer than calls made using traditional telephone service as MAGi VoIP service E911 services utilize a direct connection between MAGi's softswitch and MAGi's traditional voice switch provider before the call is transferred to the applicable Public Safety Answering Point (PSAP)
